Pediatric Advanced Care Team

When a child or adolescent has a potentially life-limiting condition families want to know that they are receiving the best care possible. The Paediatric Advanced Care Team (PACT) works with your existing health-care providers to enhance the care your child is already receiving to make sure it reflects the values and preferences of your child and your family, and to maximize quality of life and comfort for your child.
The Paediatric Advanced Care Team at The Hospital for Sick Children (SickKids) is a team of health care professionals: Four palliative care physicians, three nurses, two grief support coordinators and one administrative assistant. PACT also draws on the special knowledge of other professionals and volunteers like chaplains, social workers, nurses, a bioethicist, physicians, pain experts, volunteers and parents.

We also work closely with our community partners including: Holland Bloorview MacMillan Children's Hospital, Community Care Access Centres, Interlink Nurses, Temmy Latner Centre for Palliative Care, and Emily‚Äôs House. Working with community and hospital team partners, the team facilitates comprehensive care planning for children throughout the continuum of the illness experience, including care within the home/community. 

Services available to families and health care teams are planned to include: 

  • inpatient, community and family consultation
  • collaborative community care
  • intensive symptom management
  • psychosocial and spiritual support
  • music  therapy
  • bereavement counselling
  • financial support. 

The Paediatric Advanced Care Team  is also dedicated to the professional development of health care providers and volunteers who care for dying children and their families, high quality research and evidence based practice and advocacy on behalf of children and families who face pediatric life limiting illness and conditions.
